The information provided in this application will appear on your IPC. To complete the application, you will need: your member ID, the expiration date of your membership, a photo of your logbook for proof of certifications, a current ID photo. Upon completion of this application, please place an order in the US Sailing Store here (Step 2): http://shop.ussailing.org/international-proficiency-certificate.html. You must complete both steps in this process to obtain an IPC. Please note that average turn-around can be up to 3 weeks depending on demand.

If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture.
