Please note: Use this form to request a background check for initial or continued certification by US Sailing.

If you are uncertain whether you have a current background check, please visit your SOARS log and look under "Current Certifications and Tests" on the Main Menu or on the Annual Report page. Look for "Background Check" and the issue and expiration dates.
